Model release forms must be obtained from people or groups of people you are photographing or whose voice(s) you are recording. This form is required to use their image or voice in any medium for educational, promotional, advertising, or other purposes. One form is specifically for minors.

Areas of the model releases can be customized for your specific needs. Department/division, building and room, school, as well as address can all be modified to reflect your office.
